The form asks for your name, email address, and message. We use this information to receive and respond to your inquiry. Submitting a message does not subscribe you to a mailing list.

Please don’t include personal medical information. The form is for general inquiries and is not an urgent-help or clinical-care channel.

How your message reaches us

Cloudflare hosts the website and processes form requests. Postmark delivers form messages to COCAP’s email inbox. These providers process information needed to operate their services, which can include technical information such as IP addresses and delivery records. The website does not maintain a separate database of form submissions.

The website does not use advertising trackers or set analytics cookies. Hosting and security services may process technical information to deliver and protect the website.
